Why do all incoming calls go directly to voicemail?

Question

I am using (trying to) Google Chrome with the Hangouts extension.  I receive no indication of incoming calls..unaware someone's calling me until I get the email notification and the (sometimes) voicemail. Those who've called report that the "operator" says I'm unavailable?

Ideas?

Are you referring to your Google Voice incoming calls, or Hangout calls?
